Parents who are interested in volunteering their time at Our Lady of Calvary are always welcome. According to state law and the Archdiocese of Philadelphia, if you are volunteering to be a chaperone on your child’s class trip, or are going into the classroom to help a teacher (once or twice a school year), you need the following clearances:
PA criminal background check- to be completed at
PA child abuse clearance- to be completed at
Mandated form- found at the school website under Protecting God's Children / Safe Environment. This states that you are aware of the fact that you are now a mandated reporter and must report any suspected cases of child abuse.
Disclosure form- found at the school website under Safe Environment. This form states that you have lived in the state of PA for the past 10 years. If you have not lived in PA for the past 10 years, then you are required by law to get fingerprinted. See directions below to apply.
5. If you volunteering as a lunch moderator, classroom helper (in the classroom on a daily or
weekly basis), a scout leader, or CYO coach, you need the above 4 clearances, as well as the
Safe Environment class Part 1 (held across the Archdiocese throughout the year).
As a volunteer, all background checks are free.
(except for fingerprinting) You must register for the class.

Create a user ID and password
Choose where to take the class
After you take the class, please send in a copy of the certificate of attendance
Part 2 is an online Mandated reporter training.
This can be done by logging on to your childyouthprotection.org account and clicking on Safe Environment Part II v2: Mandatory Reporting of Suspected Child Abuse. At the end print out the certificate of completion and send a copy into school.

Below are the step by step directions for filing for the PA Criminal Background Check, and the PA Child Abuse Clearance.
How to apply for your Criminal Background Check:
Click on New Record Check (Volunteers Only)
Scroll to the bottom of the page, check off the box in the Volunteer
Acknowledgement Section, and then click on the Accept button.
Complete the form.
Volunteer Organization Name: Our Lady of Calvary School
Organization phone number- not necessary
First name (middle name is not necessary)
Last name
Address, city, state, and zip code
E-mail address, and then confirm the address
Phone number
Once completed, click Next.
5. Review the personal information, then click Proceed.
Fill out the Record Check Request Form
Click on Enter This Request.
Click Finished.
Click Submit
Wait a few seconds, and you should have your results, unless it reads PENDING.
Click on the Control number.
Click on Certification Form
A box will pop up- click OK, and then the form appears.
Hit PRINT, and print out two copies, one for you, and one for the school.
How to apply for Child Abuse Clearance:
Create an account and Keystone ID (do not lose)
The site will send you a password to your email. Once in your email, you can
access the child welfare portal from there.
Login with the password given.
A security code will be sent to you- apply the code
Continue to the page where you create a clearance application
Choose the application purpose- if for volunteering, it is the first one
Fill in category(other) and agency name(OLC)
Continue to fill in all information needed
10. Towards the end, an authorization code is NOT required
11. An email will be sent to you once the application is processed. You will have to go back into your Keystone account and then print print out 2 copies of the clearance.
Send one to school and keep one.
